Currently, we participate with the insurances listed below. We may participate with some programs and not others within a particular company. This list will change from time to time. Please contact our office to confirm whether we participate with your insurance (please have your insurance information available when you call). Many insurances that we are not contracted with will provide coverage for procedures performed. We will submit the necessary forms and information to receive the appropriate reimbursement. In this case, we will ask for a deposit at the time of the appointment. After the insurance has paid, you will either receive a refund from our office or a bill for an outstanding balance. Prompt payment of any outstanding balances is appreciated and will help avoid any collection action.
